That's a cute suggestion.  I did try the pdftohtml, but the results were
actually less helpful than the simple pdftotext.

The html has been pretty much completed, but it was largely retyped and
reworked by hand.  Even cutting and pasting from the output of pdftotext
was simply more bother than it was worth.

The pdf was worthless as a kick-start.
